ICAN, located in San Jose, CA, is looking for a bilingual and bicultural English/Spanish or English/Vietnamese Case Manager to support our HPS (Homelessness Prevention System) Program. The HPS Case Manager will be working with low-income families to prevent or end homelessness by: 1) screening, assessing, and conducting intakes from prospective participants; 2) enroll families into the HPS program with all required documentations collected and completed; 3) provide enrolled families with temporary financial assistance; 4) provide families with strength-based case management to help them gain or maintain permanent housing; 5) connecting families to othercommunity resources.
